New young carers referral form

The Integrated Front Door (IFD) have recently launched a new referral form for young carer assessments. Previously, all referrals for young carers came through the general safeguarding referral form and feedback received indicated that this caused confusion and it asked a lot of questions not relevant to young carers. This new referral form has been designed with young carers, the young carer assessors and consultation with schools to ensure that the form enables the right information is received and is more user friendly. Wiltshire Council offer a range of support for young carers and the assessment is the first step to ensuring they get the right support.

Wiltshire Council's Voice and Participation Team offer excellent support to secondary schools to achieve the Young Carers in Schools Award, a nationally recognised award designed to improve and recognise schools good practise and ability to meet the needs of young carers. The Voice and Participation Team can work with any secondary school who are looking to improve their provision for young carers and work towards the award.
