Youth @ Heart

We are a dedicated Youth Charity working with children and young people, set up to empower, inform and support young people aged 15-24 and their families, living with congenital heart disease  (CHD) across the South West and South Wales, with Wiltshire sitting within this reach.

CHD is the no.1 birth defect in the UK, affecting 1 in 100 babies born, with an estimated amount of those affected in our reach areas aged 15-24 is 6220 young people.

The Charity has a Community, Youth & Family worker who support young people and families across communities in the South West/ South Wales. The Charity, also works in partnership with the NHS Bristol Heart Institute & Bristol Hospital, where many young people go for treatments, holding an ‘honorary’ contract for its ACHD Youth Worker, it enables our Youth Worker access to the Bristol Hospital and Children’s hospital wards where they are based. This combination gives us unique support opportunities to young people affected on and off the wards, helping continuity of care when back in the community.

Our direct work with young people includes: 1-2-1 youth support for those aged 15-24 with CHD, in person, on social media, on the phone and online, in hospital, in the community, special events and activities, to attend appointments, plus a regular monthly ‘drop in’ which is open access, targeted youth sessions open to all youth with CHD across the South West.
